Referral Circumstances:
A church friend concerned about primary caregiver burnout contacted Oxford with the approval of the primary caregiver. Upon contact with the primary caregiver/spouse it was learned that the client had a LTC insurance policy available to cover Certified Home Health Aide service.

Care Plan:
Certified Home Health Aide provides assistance with activities of daily living 8 hours a day, 7 days a week, adjusted as needs indicate. Assistance is provided with bathing, grooming, dressing, toileting reminders, incontinence care, safety supervision, meal preparation, light housekeeping, skin and vital signs monitoring as well as providing/supporting appropriate social and recreational activities. RN supervisory visits are provided every 60 days and more often as necessary.

Brain Injury
Referral Circumstances:
Single working mother contacted Oxford HealthCare needing assistance for care for her son who was recently discharged from a rehabilitation facility.
Demographics:
27 yr. old male living in his mother's home
Diagnosis:
Post-motor vehicle accident traumatic brain injury
Complicating Factors: Choking risks, very limited speech, reduced impulse control, maximum assist for transfers
Care Plan:
A Certified Home Health Aide provides care in the home 8 hours a day, 4 days a week. CHHA's assist with activities of daily living; including, bathing, dressing, incontinence care, repositioning, safety supervision, transfers, laundry, meal preparation, vital signs monitoring, skin monitoring, feeding assistance and light housekeeping. RN supervisory visits are provided every 60 days and more often as needed.
