Case Manager
Job Description
POSITION SUMMARY
Educate, analyze, plan, coordinate and oversee service delivery. Evaluate consumer outcomes resulting from the care received.
ESSENTIAL FUNCTIONS:
- Identify consumer needs, goals, abilities, resources and supports as the basis for service planning by facilitating interdisciplinary team processes
- Ensure ongoing coordination of available community resources, including non-Medicaid services and informal supports, for which the consumer is eligible and/or has need, in the service plan and team process.
- Prepare and submit a comprehensive, written service plan for each CHC consumer.
- Implement certified ADvantage service plan for each consumer.
- Monitor the delivery of services as authorized in the certified ADvantage service plan.
- Identify high risk situations and special needs of ADvantage consumers using high risk criteria
- Orient and educate consumers regarding long term care programs.
- Modify services to meet needs of consumers, when necessary, without disruption of services or threat to the consumers= health and safety and ensure plan of care is moving consumer toward independence.
- Review and monitor direct service provider emergency procedures to protect the health and safety of the consumer.
- Ensure confidentiality according to all relevant laws and regulations.
- Maintain adequate records to document the performance of case management activities and responsibilities.
- Justifies expenditures and monitors, monitors, and contains cost of Plan
- Assist in implementation of in-service education for field employees.
REPORTING RELATIONSHIP
Reports to Case Management Supervisor
POSITION REQUIREMENTS
Education/Experience:
Certified ADvantage Case Manager. Bachelor’s degree in Social Services or a field related to aging or health services; or RN or LPN currently licensed in Oklahoma. Position requires a minimum of two years experience in health or social service related field. Experience may be substituted for educational requirements.
Skills/Abilities:
Strong assessment skills, resourcefulness, customer service oriented, strong communication skills, cost analysis skills, supervisory skills, and ability to prioritize.
